Gold, white and yellow lights sparks

A home care firm whose technology improves care in the home received the Best in Show award in the 2022 McKnight’s Excellence in Technology Awards + Summit.

ConcertoCare on Tuesday took home the top prize — among all the winners in the Home Care, Long-Term Care and Senior Living tracks — for its proprietary software, Patient3D. The goal of the software is to close patients’ existing care gaps. To achieve this aim, Patient3D helps ConcertoCare’s teams with three tasks: assimilate large quantities of complex data from multiple sources; analyze data to generate insights around a patient’s needs; and manage the care delivery workflow and streamline communication between care teams, patients and caregivers.
