Keeping track of a patient’s health can be a difficult task, especially for healthcare professionals who are stretched thin. Elara Caring is working to make real-time data accessible to these professionals with its proprietary ElaraConnect care platform. For its efforts, the organization received Gold in the Innovator of the Year category in the Home Care track of the 2023 McKnight’s Excellence in Technology awards. 

ElaraConnect consists of four programs: ElaraConnect Engage, a change-in-condition program for skilled home health patients and personal care clients; ElaraConnect Monitor, a remote patient monitoring program targeting the patient population with the highest risk of hospitalization; Check-In, a post-discharge follow-up program, which monitors its prior skilled home health patients after discharge; and Transition, which identifies end-of-life needs for skilled home health patients and transitions them to hospice services, as appropriate.

In 2022 alone, ElaraConnect helped Elara double the number of average home health patient touch points while reducing costs by 25% and cutting its hospital readmission rate by more than 50%. The same year, Elara’s patients, clients and families awarded Elara a Net Promoter Score of 92 — an indication of customer appreciation and loyalty unheard of in the home health arena.
