In the midst of the COVID-19 pandemic, seniors living at home were especially vulnerable to isolation and loneliness. Seeing a problem, Emerest Connect sought to find a solution using its home care technology. Its efforts resulted in a Gold award in the Tech Partner of the Year category in the Home Care track of the 2023 McKnight’s Excellence in Technology awards. 

Using data and analytics, Emerest Connect utilizes a proprietary algorithm and machine learning to create patient groups based on clinical and behavioral health data, personal backgrounds and social determinants of health. Through that process, healthcare professionals are better equipped to resolve client issues such as loneliness and isolation, depression and anxiety, as patients are grouped appropriately to improve their social connections.

A core part of its offerings is a studio that provides engaging, educational and enjoyable content to enhance social interaction for elderly and homebound patient groups. As a two-way technological tool, patients can directly engage and communicate with their peers, caregivers and expert healthcare professionals. Emerest Connect partners with home care and specialized care agencies to enhance their services across behavioral health, social work, medication management, remote patient monitoring, nutritional health and on-call doctor visits.
