Dice form the expression "rebrand".

It’s official: Humana’s home health unit Kindred at Home is now known nationwide as CenterWell Home Health. Louisville-based Humana completed the third phase of the CenterWell rebranding Thursday at all home health locations in 38 states.

The first phase of the rebranding effort began in March in seven states, followed by rebranding in 14 additional states in early June.

The company called the transition to CenterWell Home Health a major step in the integration of home health operations into Humana.

“Integrating home health operations into Humana’s home solutions business remains on track as we continue to build and scale our value-based home health offering to improve patient health outcomes nationally,” Andy Agwunobi, MD, MBA, president of Humana Home Solutions, said in a statement. “The rollout of the CenterWell Home Health brand reinforces our efforts to expand our home health capabilities to meet the needs of our patients.”

The CenterWell brand is an effort to bring Humana’s three main healthcare services – home health, senior primary care and pharmacy – under one single name.

Last month, Humana divested its majority interest in the personal care and hospice divisions of Kindred at Home. The company announced the sale of its 60% stake in that unit in April to private investment firm Clayton, Dubilier & Rice for $2.8 million. The new stand-alone company is known as Gentiva.
