A new partnership between value-based kidney care leader Strive Health and accountable care organization Evolent Care Partners will help physicians in Michigan and North Carolina better manage their patients’ chronic kidney disease and end-stage renal disease.

“We are equipping primary care providers with the resources they need to identify patients at high risk for kidney disease early in the process, so they can take preventative steps to improve outcomes and lower the cost of care,” Strive Health Senior Vice President Farhad Modarai, M.D., said in a press release. 

The providers served through the new partnership manage care for up to 1,200 beneficiaries living with advanced chronic kidney disease and end-stage kidney disease. The companies said primary care providers play a critical role in early identification of the disease, which cost Medicare more than $87 billion in 2019.  

Strive will provide Evolent Care Partners’ provider network with access to data and technology tools, administrative support, management expertise and an interdisciplinary care team that includes care coordinators, nurse practitioners, dieticians, pharmacists and licensed clinical social workers. 

Last year, Strive launched Innovative Kidney Care with health insurer Anthem and medical technology company Outset Medical in an effort to make home dialysis more accessible. The coalition is urging the Centers for Medicare & Medicaid Services to break down the regulatory barrier to in-home dialysis. Last month, the group urged CMS to allow dialysis equipment manufacturers to train patients to dialyze at home. 

Nearly 40% of Americans 65 and older have chronic kidney disease. It afflicts more than 37 million people.
