Houston, TX-based Rainbow Health has launched a new solution aimed at simplifying the clinical logistics for home healthcare.

RainbowCare House Calls Dispatch Solution helps route physician groups, health systems and payers to patients’ homes to deliver in-person care or through telehealth.

“At-home care programs are on the rise and it has become vital for healthcare organizations to utilize digital health solutions that aid in addressing the clinical and non-clinical outcomes of high-need patients,” said Ayushi Patel, Director of Strategy and Innovation at Rainbow Health parent company Mobisoft Infotech.

RainbowCare House Calls comes as the Centers for Medicare & Medicaid Services is rolling out two home-based programs, the Global and Professional Direct Contracting (GPDC) Model and the Acute Hospital Care at Home program. GPDC is a risk-bearing program emphasizing preventive and chronic care, while Acute Hospital Care at Home delivers hospital-quality care at home. 

RainbowCare House Calls Dispatch Solutions helps providers locate patients’ homes and equips them with automated, point-of-care documentation tools. Healthcare providers can also perform social determinants of health assessments to identify risk factors in the home and gaps in care that might be filled with benefits provided under health insurance plans or home-and-community-based services.
