A new collaboration between tech-based home care firm Signify Health and healthcare service firm Cardinal Health could bring medication management services to nearly 2.3 million people in their homes.

Signify Health’s mobile network of 11,000 credentialed clinicians will use Cardinal Health’s clinical care platform Outcome to help health plan members resolve medication needs through in-home health evaluations. Signify Health estimates the average older adult is prescribed up to 13 different medications. That can be complex to manage and put the patient at risk. The two companies said allowing seniors to engage with clinicians in their homes and ask medication-related questions could help reduce some of that risk. 

“From our experience conducting medication reviews in thousands of homes each day, we understand the importance of making medication management more convenient, accessible, and connected to daily life,” David Pierre, Signify Health chief operating officer, said in a statement. “We look forward to joining forces with Outcomes to bring this vital service to improve medication safety and adherence and drive more value for the people we are privileged to serve.” 

The companies said the collaboration will also provide patients a more holistic view of their health and greater connectivity between all members of their care team. 

The new partnership helps Signify Health’s parent company, CVS Health, move deeper into the home. CVS bought Signify Health last year for roughly $8 billion in a blockbuster deal. At the time the company said the deal would help it accelerate growth into value-based care.  

The collaboration between Signify Health and Cardinal Health could also benefit seniors receiving care through Oak Street Health. CVS announced the acquisition of that firm last month for $10.6 billion. That deal is expected to close sometime later this year.
